PERMISSION/FORMS TO BE COMPLETED

The school must receive new signed consent/permission each year.
Consent for Use of Student's Works/Images
A new Consent for Use of Student's Works/Images Form is required to be signed by parents and returned to the School. It is important that this form, which was sent home last week, is completed, signed and returned to the school as soon as possible.
Unfortunately, if the form is not returned we are not able to display, publish, publically acknowledge or "use" any of your student's works or images. This includes but is not limited to our newsletter, social media, newspaper articles, or in school displays/Open Days/Showcases (eg SPLICED, Gday, Office Displays, assemblies), Diocesan publications or Catholic Education Office Publications.
Responsible Use of Technology Form
it is an expectation from the Catholic Education Office that every student in our school is to read with their parents the CEDoW Student Acceptable Use of Technology Agreement at the beginning of each year. This agreement outlines the expectations regarding appropriate and safe use of technology in the education of our students. Parents and students are asked to read and sign the agreement and return it to their teacher as soon as possible before they will be able to use the laptops or iPads at school.
